Backend Engineer – Node.js, PostgreSQL & Microservices

Role Overview

Botern is a healthcare technology platform that helps clinicians capture, organize, retrieve, and use their clinical cases for learning, teaching, presentations, and research. We are building Botern as a secure, scalable healthcare SaaS product with web and mobile applications, cloud storage, structured data workflows, and AI-assisted features. We are seeking a skilled Backend Engineer to join our team and help build and scale the backend systems powering Botern.

Responsibilities

• Develop and maintain backend services using Node.js.
• Design and build scalable REST APIs and microservices.
• Design, manage, and optimize PostgreSQL databases.
• Build and maintain containerized services using Docker.
• Develop backend systems supporting web applications built with React.
• Integrate AI-assisted workflows and external AI APIs into product features.
• Build integrations with cloud services, storage systems, and third-party APIs.
• Develop background jobs, asynchronous workflows, and data-processing services.
• Optimize backend systems for performance, reliability, and scalability.
• Implement secure authentication, authorization, and access-control systems.
• Deploy, monitor, debug, and maintain production services.
• Collaborate with frontend and mobile developers to deliver features from development to production.
• Participate in code reviews and maintain clean, modular, and scalable architecture.

Requirements

• Bachelor's degree in Computer Science, Engineering, or a related field, or equivalent practical experience.
• Strong experience with Node.js.
• Good knowledge of PostgreSQL, including database design, queries, and performance optimization.
• Experience designing and building RESTful APIs.
• Understanding of microservices architecture.
• Experience with Docker and containerized application development.
• Familiarity with React and modern frontend-backend integration.
• Experience working with cloud platforms such as AWS, Google Cloud, or Azure.
• Experience integrating third-party APIs and external services.
• Experience deploying and maintaining live applications.
• Strong problem-solving skills and ability to independently take ownership of features.
• Good communication and collaboration skills.

Good to have

• Experience integrating LLMs or AI APIs such as OpenAI, Gemini, Claude, or similar platforms.
• Experience building AI-assisted workflows or structured data-processing pipelines.
• Experience with Redis, queues, event-driven systems, or background workers.
• Experience with CI/CD and automated deployment workflows.
• Familiarity with Kubernetes.
• Experience with healthcare technology, secure cloud infrastructure, or HIPAA-compliant applications.
